Assessing the safety of low dose primaquine in Plasmodium falciparum infected African children with glucose 6 phosphate dehydrogenase deficiency (PAC Study)

The aim of this study is to provide evidence for drug policy on the safety of the WHO recommended single low dose primaquine regimen when used under close to routine conditions to G6PD deficient  African children with acute uncomplicated P. falciparum. This study proposal answers the WHO’s call for more research on SLDPQ and will fill a number of substantial knowledge gaps in young African children, including the tolerability of SLDPQ at the WHO recommended dose in children with high biomass falciparum infections and often concomitant anaemia, the factors associated with the development of severe/profound anaemia, the Pharmacokinetic of Primaquine and the  prevalence rates of CYP 2D6 polymorphisms and their relationships to PQ PK.

Baseline assessment of neonatal hyperbilirubinaemia in a cohort of new-borns in Kinshasa, Democratic Republic of Congo (NNJ Study)

The primary objective is to evaluate the prevalence of neonatal hyperbilirubinaemia in a cohort of 306 in-hospital consecutive live births. The results of this survey will provide essential baseline data for the community. If the frequency of the NH and severe NH in the area is higher than routinely reported, prompt and appropriate management guidelines can be put in place to decrease neonatal mortality and neurological disabilities. At the same time, the use of better diagnostic tools for identification of NH and associated risk factors can be implemented. Likewise, a program of community engagement to teach mothers how to recognize signs of hyperbilirubinaemia can be piloted as lack of education and awareness of this condition is a well-known additional risk factor.

Evaluating the renoprotective effect of acetaminophen in pediatric severe falciparum malaria: A randomized controlled trial

The overall goal is to assess the renoprotective effect of APAP in severe malaria. Our hypothesis is that APAP will reduce kidney dysfunction by inhibiting CFH-mediated oxidative damage. The study will also assess APAP’s: (i) safety, (ii) effects on fever, coma and parasite clearance, and (iii) pharmacokinetics and pharmacodynamics.

Development project for Triple Artemisinin Combination Therapies (DeTACT)

The goal of the project is to provide the evidence and the products to support a global change in antimalarial treatment. The impact expected is progress to malaria elimination and, critically, prevention of a potentially catastrophic reversal of all recent gains in malaria control because of the spread of ACT resistance.

Two quality assured triple artemisinin combination therapies (TACTs) which have been extensively tested for efficacy, safety and tolerability in both children (mainly Africa) and adults (mainly Asia) will be delivered by the end of this project. To facilitate policy transition the project will assess the market positioning of TACTs as a new first-line treatment for falciparum malaria in both African and Asian malaria endemic countries, and model the potential health and economic impact of delaying the emergence and spread of multi-drug resistant falciparum malaria by switching first-line treatments to TACTs. This project should provide new effective and safe treatments for falciparum malaria not only in countries currently threatened by untreatable malaria (SE Asia), but also in endemic countries threatened by the importation of these drug resistant strains (South Asia and Africa).

PUBLIC ENGAGEMENT

KIMORU has set since 2014 ie five years now a service to the community. It is the passive screening and treatment of uncomplicated malaria in suburban areas of Kinshasa.

This activity is totally free and is continuous ie 7 day a week, 24hours a days since its beginning.

We are supported in this activity with SANRU/Global Fund.

By December 2018, we have screened 35,211 symptomatic children attending KIMORU and treated 23,553 for uncomplicated malaria with an ACT (Rapid Diagnostic Test positive).
